To utilize JVZooPay as an Affiliate, you are required to complete the tax forms based on your account setting as a US Affiliate or International Affiliate.

If you are a US Affiliate, you are required to complete a W-9 for yourself individually, or for your business.

If you are an International Affiliate, you are required to complete a W-8BEN if you are an individual, or a W-8BEN-E if you are an international corporation or partnership.
